RN Manager – Case Management

💰 Salary Range: $148,000 – $198,000
🛏 Scope: Over two hospitals totaling 500+ beds
📋 Reports to: Director of Case Management
👥 Team Size: Approx. 50 FTEs
About the Opportunity:
Our client, a nonprofit, faith-based teaching hospital in East Los Angeles, is seeking a compassionate and strategic RN leader to oversee Case Management operations across its campus and partner facility. This role will manage a team of approximately 50 professionals and play a key role in driving care coordination, discharge planning, and readmission reduction initiatives. Key Responsibilities

Provide strategic and operational leadership for Care Management services across two hospital campuses

Monitor departmental performance using data and key metrics to improve productivity and efficiency

Lead initiatives to reduce length of stay (LOS), minimize readmissions, and streamline discharge planning

Coach, mentor, and support a multidisciplinary team of case managers, social workers, and RN care coordinators

Ensure individualized, holistic care plans that address both clinical and socio-economic patient needs

Align care coordination efforts with Adventist Health’s quality, regulatory, and compliance standards

Identify and address systemic challenges to care delivery, developing solutions that enhance outcomes

Interpret and implement updates to case management policies, regulations, and revenue cycle processes
Qualifications
Education & Experience

Bachelor’s degree required; Master’s degree preferred

Minimum of 5 years’ experience in case management, hospital discharge planning, utilization review, or social work (preferred)

At least 1 year of leadership experience in a clinical or healthcare setting (preferred)
Licenses & Certifications

Active Registered Nurse (RN) license in the State of California (required)

Hospital Fire and Life Safety (HLFS) certification (required or must be obtained upon hire)
